Patient Introduction & Informed Consent

 

Energy healing, Reiki and Intuitive Energy session are energetic healing methods, which involves the laying on of hands during in-person sessions and an energetic connection for both in-person and distance sessions. I understand that I will experience a series of hand positions on and/or above my body and be fully clothed during the session.

All therapies described above are complementary therapies and have the potential to create both desirable and undesirable effects. Such effects can include the following: allergic reactions/sensitivities/lifestyle changes.

If you are pregnant, have a heart condition or any other health condition please make this information known to Sofie prior to your treatment.

Please note, Sofie is not a medical doctor and does not diagnose or prescribe any medications.

 

Life Coaching

You are responsible for your own life and decisions. Sofie Simonsson is not qualified to give Medical, Legal, Career or Financial Advice, and takes no responsibility for individual interpretations.

 

Meditations & Activation

When you listen to a meditation or activation you should not operate any machinery or be driving. Meditation and Activations must be in a safe space. You are responsible for your needs regarding your own health and health state. 

 

Privacy Policy

Sessions are completely confidential, including all information provided to Sofie in relation to sessions prior, during, and after our appointments. Please note that by law, Sofie is required to seek assistance if she believes that your life or someone else’s life is in immediate danger. Your personal information will be stored securely and will not be shared with any other party without your prior consent other than when required to do so as noted above.

 

Payment Policy

Payment is required at the time of booking. 

Appointment Cancellation Policy

I understand that 24 hours notice is required when cancelling an appointment. I also understand that the full cost of the appointment will be charged if I do not cancel 24 hours prior to the appointment. 

 

 

Your Commitment

I accept full responsibility for my progress and results, all choices that I make, actions that I undertake and suggestions that I choose to follow as a consequence of my conversations and sessions with Sofie. I understand that I am the only person who can take responsibility for my own long-term wellbeing.

 

I will not be under/or use any form of mind-altering substances within at least 24 hours of my appointment knowing that I will be turned away from a session if under the influence of a substance, and no refund will be given.

 

I have read and understood the information on this consent form. All information provided herein is true and correct. I hereby consent to treatment. I hereby request and consent to the performance of Life coaching and Reiki treatments and other procedures by Sofie Simonsson.
